Hi, my name is Jacobus and I am the owner/service provider of Acorn disability services. I am originally from South Africa, and have a passion for people, meeting individuals and getting to know them. My extensive client services experience will ensure that only the very best care is provided.

Acorn Disability Services is an owner operated support provider, ensuring client centered service. I operate on a one to one basis, with no middle man. Your contact person at Acorn Disability Services is the person who will be providing care, book appointments, provide assistance and who will be there each step of the way, based on the care required.

Acorn Disability Services offers assistance to Plan Managed and Private managed participants.

Areas serviced are Toowoomba, Gatton, Oakey, Dalby, Pittsworth.

NDIS Service Provider registration is currently in progress. Rates charged are based on the NDIS pricing guide, ensuring fair fee management. Each client will receive a Service Agreement and Schedule of support, as well as a Participant Handbook.

Services provided:

Assist with personal activities: Assistance is provided with playing games, going for walks, reading to a participant, doing any hobbies, organizing rooms/shelves, etc

Assist with Travel/Transport: Transport can be arranged for: doctors appointments, meetings, work, any other appointments, sporting events, restaurants, movies, etc

Daily Tasks/Shared Living: Assistance provided includes meal preparation, helping with showering/bathing, getting dressed, personal hygiene (clipping of nails, brushing hair, shaving, toileting) and any other assistance required.

Household Tasks: Assisting with cleaning house, vacuuming, washing floors, wiping counters, doing laundry, doing dishes, cleaning of shelves and fridges, and any other h....

Respite
Lander Support offers respite care to individuals with disability and their carer/s. Whether you and your carer require a short-term break, or an extended time apart, respite care ensures you both receive time to be independent of one another. In addition to short- or long-term options, respite care can be made available in both planned and emergency situations.
